We are currently seeking a Senior Mental Health Specialist to join our esteemed institution at our Montefiore UBA-WeCARE Program. WeCARE provides a continuum of services to help HRA referred cash assistance clients with medical and/or mental health conditions that affect their employability to attain their maximum levels of self-sufficiency. Individuals referred to WeCARE receive medical and psychosocial assessments to identify possible clinical conditions and social barriers that may affect their ability to participate in work-related activities. Based on the results of the assessments, a determination of an individual's functional capacity (FCO) is made and a customized service plan by our team is developed to provide a range of services tailored to meet a client's particular needs.

Key Responsibilities:

1. Interview clients, review records, and confer with other professionals to evaluate individuals' mental and physical condition and to determine their suitability for participation in a specific program track including Wellness, Federal Disability, employment with accommodations, or employment without limitations.

2. Conduct the Social Security Administration Sequential Evaluation as part of the assessment process.

3. Develop client treatment plans based on research, clinical experience, and client histories.

4. Review and evaluate clients' progress in relation to measurable goals described in Individual treatment plans during re-assessments.

5. Review and evaluate client Functional Capacity Outcome during re-assessment when new medical and/or behavioral health information is provided

6. Initiate referral to psychiatrist, psychologist, review physician, physician assistant, CASAC or other health professionals as indicated during the assessment in order to complete the assessment in compliance with HRA policies and procedures.

7. Additional duties as needed to ensure compliance with HRA and UBA's expectations. Duties: - Conduct comprehensive assessment - Determine Functional Capacity Outcome (FCO) - Conduct clinical re-assessments - Complete assessment documentation and review the results/outcome with the individual, whenever possible this process should be completed on the same day as the assessment.

Required Knowledge:

1. Medical/health diagnoses and impact on individual's functional capacity.

2. Behavioral health diagnoses and impact on individual's functional capacity.

3. Knowledge of medications, side effects, and impact on individual's functional capacity.

4. Requirements to qualify for Federal Disability Benefits.

5. Able to use multiple software systems to review and assess medical and behavioral health conditions as well as document assessment and outcomes in electronic medical records.

License/Certification: * LMSW, CRC, LMHC, or LCSW (Required)

Job Type: Full-time Salary: $67,000-$93,000
